Tips & Tricks Before Beginning the Application

If you feel overwhelmed, take a break. Save your progress and come back to it. A link will be sent to the email address you provide in the application that will allow you to pick up where you left off.

We strongly encourage applicant families to keep a copy of provider records they personally gather for placement consideration. Many agencies require similar information prior to considering a child for placement. Having a copy on hand will expedite the application process with another agency, if necessary. 

Complete the application in its entirety. Missing information could cause application processing to be delayed or declined. 

Carefully monitor the email address provided in the application once it has been submitted. Required forms, provider records, and documentation requests will be sent and received via email throughout the process.

Gather required documents and information prior to beginning the application.

Information you will need:

A.  Family member’s names, ages, birthdates, Social Security numbers, health insurance information, etc. 

B.  Names and contact information for your child’s current and previous healthcare providers, therapists, schools, placements, etc. to complete Release of Information* forms. 

* Release of information forms are required for each of your child’s providers.

  • Providers:
    • Mental Health: Psychiatrists, psychologists, counselors, mental health hospitalizations, evaluations, and results of pertinent testing. 
    • Healthcare: Physicians, specialists, vision, dental, hospitalizations, etc.
    • Educational: All schools, including placement testing, IEP or 504 plans, disciplinary records.
    • Prior Placements: Foster care, voluntary placements including other residential care or children’s homes.
  • Release of Information forms are automatically populated as the placement application is filled out and will be emailed to you for your signature once the application is submitted.
  • You may be asked to present the release forms directly to your child’s providers.
  • Please download and print as many copies of the release as necessary. 
  • You may submit provider records to the placement team personally or they may be sent directly from providers.
  • Failure to sign, submit, deliver, or respond to requests for information are the primary reason for application consideration to be delayed. 
 
We understand the sensitive information you’re asked to provide about your child can feel very invasive; however, answering honestly is imperative to a successful placement outcome. You will have an opportunity during the placement process to discuss and elaborate on information in the application with a placement representative over the phone. The placement team takes into consideration your child’s emotional and behavioral needs, the experience and background of the homeparents whose home they are being considered for, the ages, genders, behaviors, and needs of the other children residing in the home, and much, much more. Honest, forthcoming responses allow the placement team to prioritize the safety and wellbeing of all members of the home. Neither you or your child will ever be judged for information shared in the placement application, and your security is of the utmost importance to our ministry.
